Flu Vaccine Consent Form

To make your vaccine visit go faster and to reduce handling of paper work, please download and complete a consent form in advance. Bring completed forms to the clinic with you. A separate form is required for each person being vaccinated.

Bring your photo ID with your current address, and Complete the top portion of the Vaccination Consent for Influenza form and bring it with you. DO NOT sign the paper until a Medcor provider has reviewed it. The Vaccine Information Statement is for your review, and no action is needed.
